It's not the tie that gets it. It's the knee. Those recesses in the wheel allow quite a whack if your knee's up against it.
Geared like that to allow for manual trim. Big knob pops out...crank away!

Yep, I APDed on the E145 for a while. Don't think I ever saw a crew catch a trim runaway before it was a two man job to fly it.
737 runaway... simple checklist...
yoke HOLD TIGHT autopilot OFF trim disconnect OFF trim wheel HOLD FO's knee PULL INTO TRIM WHEEL TILL STATIONARY:D

Yea. I had one in the sim once when I was just a pup. pfff... I think I should have just let the damn thing crash, I was sore for a week. Roll inverted and pull. But the instructor was trying to teach a lesson about guarding the yoke and he did that in spades...

In cruise, if the trim wheel moves, something has changed enough that I need to pay attention. Mike C.
