Might be caused by sharting. Our high school metal shop teacher had a list of shop rules. One of them was never fart and walk away. Perhaps he knew the risk it posed to quality. My daughter works for a pharma firm and they tried outsourcing some of their production to Puerto Rico. They had to spend so much time keeping an eye on things that they moved it back to the U.S.
