I believe there is no meaningful penalty, far less risky than a piston aircraft. When the engine reaches TBO, it has reached its lowest core value pretty much. The gearbox is not going to be harmed. Gear box problems are almost always detected by SOAP prior to failure, so very little risk.
As a part 91 owner, you can consider buying timed out engines and doing a HSI only on them and flying them another 1800 hours. Then, at 7200 hours, they are due another HSI and another gear box inspection.
As the -1 owners trying to extend their flying without overhaul, you will see more and more of this.
