Only the bolded part is releated to LSA flying, and all it says that you cannot know of "any medical condition that would make the person unable to operate the aircraft in a safe manner". So if you, through self-certification, determine that you are able to operate the aircraft in safe manner, you are good to go. No, the 15 listed disqualifying conditions apply to both types of operations. Not limited to those 15 for either type but those are just the obvious ones.

Disqualifying medical conditions are disqualifying you from eligibility for a medical certificate. They don't automatically disqualify you from being "able to operate aircraft in safe manner".
