Almost everyone's old passwords are in large data dump files online. Some darkweb sites sell them for $1 per 1000+ bulk rate, but mostly they are free for anyone to download. Most people don't change their passwords frequently (or at all) and re-use the same passwords across multiple sites.

So when Yahoo gets hacked and 500 million users' passwords, email addresses, and other personal information is released any unsavory folks who wish to have that information, it is simply a matter of trying those passwords out on other sites to see which ones use the same password. Then they exploit that trust relationship they just gained by peddling used avionics to unsuspecting buyers.

Severity should go to anyone with a crap password unprotected and easily hacked. Any website that allows multiple failed attempts and doesn't lock the IP is out of date too. Any failed user attempt that doesn't send a warning back to the email address is not using current practices. Simple stuff stops it. $20 worth of programming is all it takes.
